August 24, 2015 - Imagine Medispa, LLC, Charleston, Kanawha County, CON File #14-3-10298-P, for the development of a clinic in Charleston, Kanawha County, for the provision of physician supervised weight loss, skin care services and retail skin products; capital expenditure: $7,500.
August 24, 2015 - Monongalia General Hospital, Morgantown, Monongalia County, CON File #15-6-10518-X/E, for the acquisition of a permanent Positron Emission Tomography/Computed Tomography (PET/CT) unit and the provision of fixed site PET/CT services; capital expenditure: $2,045,000.

August 26, 2015 - Willow Creek Health Clinic, Premier, McDowell County, CON File #15-1-10571-X, for the establishment of a private practice medical office to be located in Premier, McDowell County. The practice will be operated in collaboration with a physician and will focus on providing primary health care; capital expenditure: $300,000.
August 27, 2015 - Thomas Memorial Hospital, South Charleston, Kanawha County, CON File #15-3-10578-X, for the relocation of TMH's outpatient cardiac rehabilitation services from 2605 Kanawha Avenue, S.W. to the new address of 4610 Kanawha Avenue, S.W., South Charleston, Kanawha County; capital expenditure: $100,000. August 27, 2015 - Boxwood, LLC, Charleston, Kanawha County, CON File #15-1-10508-NH, for the construction of a replacement facility for Mercer Nursing and Rehabilitation Center, LLC, an 83 bed nursing home located in Bluefield, Mercer County, West Virginia; capital expenditure: $16,000,000.
August 27, 2015 - Keyser Center LLC, Kingwood Center LLC, Martinsburg, Center LLC, and Rainelle Center LLC, Charleston, Kanawha County, CON File #15-4/6/8/9-10525-A, for the acquisition of the licensed operations associated with the Heartland of Keyser, a 122 bed nursing home located in Mineral County; Heartland of Preston County, a 120 bed nursing home located in Preston County; Heartland of Martinsburg, a 120 bed nursing home located in Berkeley County; and Heartland of Rainelle, a 60 bed nursing home located in Greenbrier County, West Virginia; capital expenditure: $10,200,000. August 26, 2015 - Domestic Violence Survivors Support Group, Inc. d/b/a Domestic Violence Counseling Center, South Charleston, Kanawha County, CON File #11-3-9397-X/BH, for the provision of intensive outpatient behavioral health counseling services designed to provide culturally appropriate mental health counseling and therapy services for domestic violence victims for men, women, and children suffering the effects of domestic violence trauma; no capital expenditure.

(SpinCo), Solomon 353, LP n/k/a Care Capital Properties, LP and Solomon 353 GP, LLC n/k/a Care Capital Properties GP, LLC, Chicago, Cook County, Illinois, CON File #15-4/5-10476-A, to spin-off most of Ventas' post-acute/skilled nursing facility portfolio into a separate publicly traded real estate investment trust positioned for growth, resulting in the transfer of interests in the entities that own the land and buildings of the two West Virginia skilled nursing facilities: Nicholas County Nursing and Rehabilitation, a 97 bed nursing home located in Richwood, Nicholas County and Worthington Nursing and Rehabilitation Center, a 105 bed nursing home in Parkersburg, Wood County to SpinCo; capital expenditure: $36,100,000.
