The West Virginia Parkways Authority, hereafter "Authority", is accepting bids for supplying all materials and installing a fabric covered arch style roofing system for a salt storage building that is being constructed at the Beckley Maintenance Facility located at Exit 44 along the West Virginia Turnpike.

All work under this contract shall be completed by September 19, 2015. The start date for the roof installation is on September 1, 2015. In the event that work can begin sooner than September 1, 2015, the WVPA will notify the contractor seven (7) days prior to the completion of the concrete building.

Proposals will be received at the office of the West Virginia Parkways Authority, Administration Building, Charleston, West Virginia until 3:00 PM February 19, 2015, at which time and place said proposals will be publicly opened and read. The Town of Clay is accepting Statements of Qualifications from qualified persons or firms to provide engineering and construction management services which will include a preliminary engineering report, design services including drawings, preparation of construction plans and specifications, bid documents, construction administration/ management and observation services for the Blue Knob Waterline Extension.

The Town reserves the right to select an individual person or firm or multiple persons or firms to complete these projects based on their rankings and their rankings. The Selection Committee will select firm(s) and negotiate the fee for design of the projects

Procurement of said services would be in accordance with Chapter 5G-1-3 of the WV Code. The Selection Committee will include members from the Town Council, the Mayor or the Recorder.

The project is proposed to be financed in part by grants from the U.S. Department of Housing and Urban Development (HUD), the Appalachian Regional Commission (ARC), the West Virginia Infrastructure and Jobs Development Council (WV IJDC), the U.S. Department of Agriculture - Rural Utility Service (RUS), and the West Virginia Development Office and shall be referred to as the Blue Knob Waterline Extension Project.

Proposals must include:

1)Experience of the firm with HUD-funded projects.

2)Experience in architectural design and construction management of this type of project.

3)Qualifications of personnel directly involved in the project

4)Dedication of time toward the project

5)Related prior experience

The selected person or firm will be required to comply with Title VI of the Civil Rights Act of 1964, Executive Order 11246, Section 109 of the Housing and Urban Development Act of 1974, Section 3 of the Housing and Urban Development Act of 1968, Conflict of Interest Statement and Access to Records provisions, Architect's Certification and all other requirements as they relate to HUD-funded projects.

Notice is hereby given by the West Virginia Department of Transportation, Division of Highways, that consultant services will be utilized for the performance of various tasks to be performed under the following Statewide (On Call) Agreements:

Statewide Cultural Resource Services - Consulting services will be utilized for cultural resource services for compliance with Section 106 of the National Historic Preservation Act. The work will generally consist of Section 106 documentation and related work including identification surveys, evaluations for National Register eligibility, and data recovery projects for historic and archaeological resources.

Statewide Natural Resource Services - Consulting services will be utilized to provide Natural Resource work for NEPA compliance relating to roadway and bridge projects in West Virginia. The work will be performed statewide upon specific requests by the Division of Highways and may include, but not necessarily be limited to; stream and wetland studies, state and federal listed endangered species investigations, non-listed species surveys and biological surveys, and other related services.

Method of payment shall be Lump Sum per assignment.

Multiple firms may be selected for this work.

Firms interested in being considered for any or all of these projects must submit a "Letter of Qualification" for each project and one unpriced prospectus to Mr. Michael D. Lilly, P. E., Section Head, Technical Section, Engineering Division, Division of Highways, 1334 Smith Street, Charleston, West Virginia 25301 or preferably by email to doh.consultantservices@wv.gov prior to 4:00 p.m. on February 9, 2015.

As a minimum, the prospectus shall include the following evaluation factors:

1.Professional qualifications necessary for consideration and satisfactory performance of the required work shall be provided for that person-in-charge of overseeing the work. That person shall be located within the office where the work is to be performed, and shall demonstrate sufficient past experience performed for the State of West Virginia, Department of Highways related to design and plan development to be provided. This individual shall be readily accessible to the Department, preferably located in-state.

2.Consideration shall be given to specialized experience and technical competence. Specialized experience shall focus on the qualifications of the staff working on the project with emphasis placed on individual specialized expertise required for the project. As a minimum, the firm shall provide a Résumé of those individuals who will be committed to the needs of the project. Strong considerations will be based on the professional and non-professional staff specialized experience and technical competence stationed within the office where the work is to be performed.

3.Capacity consideration shall be made on the firm's ability to accomplish the work within the Department's required schedule time frame; personnel dedicated to perform such work.

4.Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules.

5.Location shall be based on the geographic location of the Consultant's office relative to the project site along with past experience and knowledge of the area where the project site is located with the significant importance being given to accessibility of the office location relative to the Department.

In addition to the above evaluation factors, the prospectus shall include, as a minimum, the following items:

1.A completed copy of the Division's Consultant Confidential Qualification Questionnaire dated January 1, 2015 or later, if not already on file. It must specifically indicate the staff levels by classification located in-state vs. out-of-state.

2.Location of office or office(s) in which the work, or part of the work, will be performed.

3.Identification of subconsultants by name as to the type of work anticipated to be subcontracted or performed, if proposed as part of the design team.

4.Indication of whether a cost accounting system has been maintained and is in effect such that it is capable of segregating and identifying accumulating costs for each job that is performed under cost type projects.

5.Completed copies of the Division's "Consultant Short List Selection Criteria Technical Evaluation" form limited to a single page of data. One loose leaf copy of form is to accompany the bound unpriced prospectus. Cross referencing on this form to other parts of the prospectus is not acceptable.

A single "Letter of Qualification" for each project and one unpriced prospectus are required and must contain all of the above information required to be considered. Failure to provide the required information and/or insufficient information will cause disqualification and the prospectus will be returned. All evaluation factors as noted herein before will be given equal consideration in the selection process, unless a project specific weighting of the evaluation factors is shown in the Notice for Consulting Services.

Prior to entering into any contract negotiations, the selected firm and any subconsultants shall submit a current audited overhead rate computation performed by a State or Federal government agency or independent CPA firm. The submitted overhead audit shall contain a detailed exhibit of the computations with all applicable eliminations and references. The audit shall be done in accordance with Government Auditing Standards issued by the Comptroller General of the United States and all eliminations required by the Federal Acquisition Regulations will be made. The submitted certified audited overhead (Indirect Cost Rate) report shall include the certification, certifying that the allowability of costs used to establish the final indirect cost rate are in accordance with the FAR cost principles, or a verification indicating this certification was previously submitted and accepted by the cognizant agency. This requirement will be waived for the selected firm or any subconsultant when its portion of the fee for the project is less than One Hundred Thousand Dollars ($100,000.00). In such instances, a company computed overhead rate may be utilized.

This project shall consist of the development of Contract Plans for the superstructure replacement of a bridge carrying West Virginia Route 2 over Proctor Creek. The existing bridge is 235 feet long by 56 feet wide and is located 0.06 miles south of Wetzel County Route 89.

Method of payment shall be Lump Sum.

A single assignment will be made for each project.

Firms interested in being considered for either of these projects must submit a "Letter of Qualification" for each project and one unpriced prospectus to Mr. Michael D. Lilly, P. PUTNAM COUNTY

Putnam Public Service District has completed the installation of sanitary sewer lines in the Route 33 areas of Putnam County.

1.All areas of construction are operational and service is available as of January 19, 2015.

2.The District will begin mandatory billing for sewer service on the final completed stage in April 2015 as authorized by West Virginia Code §16-13A9 and Rule 4.3 of the Public Service Commission of West Virginia Rules for the Government of Sewer Utilities. Billings shall be based on actual water consumption.

3.The District's tariff upon completion of all construction from the
Sewer Project is as follows:

$ 11.20 per 1,000 gallons of water used

$ 22.40 minimum bill

$ 33.59 flat rate for non-metered and users who obtain water from wells

This rate has been approved by the Public Service Commission of West Virginia, Case No. 12-1209-PSD-CN.

4.West Virginia Code §16-13A9 authorizes the District to petition the
Circuit court of Putnam County to compel connection to the sewer system.

5.Residential customers, in the final stage that is now operational, that do not connect to the sewer system will be billed on actual water consumption, up to 4,500 gallons, starting with the service dates of February 19, 2015 . These charges will reflect on your April billing. Previously completed areas are already being billed.

6.All persons in the project area who have not made a request for service with the Putnam Public Service District's must do so at this time. The main office is located at 107 South Poplar Fork Road, Scott Depot, next to the Teays Valley Volunteer Fire Department. The District's telephone number is (304) 757-6551.

7.Demonstrated successful past performance (including meeting costs, schedules, and performance requirements) of contract work substantially similar to that required by the solicitation as verified by reference checks and other means.

Each of the above subject matters will be rated in accordance with established HUD guidelines. The successful firm will be selected based on the proposal content which is deemed to be the most advantageous to the Housing Authority of the City of Williamson. The Town of New Haven, Mason County, West Virginia, is in the process of selecting a Project Architect/Engineer to provide certain architectural/ engineering services for the following project: Water System Improvements Project. Professional services will include the following items: design of water system improvements/ extension; preparation of bidding and contract documents; participation in the solicitation and evaluation of bids; assistance in obtaining easements and other property if necessary; surveying and mapping; preparation of all necessary permit applications; construction administration and construction inspection. Procurement of said services shall be made in accordance with section 5G of the West Virginia State Code. Contracts are to be awarded on a lump sum/fixed fee basis.

All engineering firms interested in being considered for this project must submit five (5) copies of their proposal detailing technical expertise, qualifications, and related prior experience. Proposals must be submitted by 4:00 p.m. (lpt) on February 13, 2015, to the following address: Town of New Haven, P.O. Box 217, New Haven, WV 25265. If forwarded by mail, the sealed envelope containing the proposals must be enclosed in another envelope with the project name and Architectural/ Engineering Services Proposal highlighted on the outside of the outer envelope and must allow for sufficient time for mailing to reach the above referenced address prior to the scheduled closing time for the receipt of proposals. The goal of the competitive process is to objectively select the firm which will provide the highest quality of service at a realistic fee. Accordingly, technical qualifications and experience will be weighed heavily. A committee of representatives as determined by the Town of New Haven shall evaluate the statements of qualifications and performance data and other material submitted by interested firms and select a minimum of three firms which, in their opinion, are best qualified to perform the desired service. Interviews with each firm selected shall be conducted and the committee shall conduct discussions to evaluate the presented information. The committee shall then rank in order of preference no less than three professional firms deemed to be the most highly qualified to provide the services required, and shall commence scope of services and price negotiations with the highest qualified professional firm for said services. Should the committee be unable to negotiate a satisfactory contract with the professional firm considered to be the most qualified, at a fee determined to be fair and reasonable, price negotiations with the firm of second choice shall commence. Failing accord with the second most qualified professional firm, the committee shall undertake negotiations with the third most qualified professional firm. Should the committee be unable to negotiate a satisfactory contract with any of the professional firms in order of their competence and qualifications, it shall continue negotiations in accordance with these procedures until an agreement is reached.

Attention is directed to the fact that the proposed project may be undertaken using various Federal and State funding sources, examples of which are: Small Cities Block Grant Program (SCBG) from the U.S. Department of Housing and Urban Development through the State of West Virginia, USDA -Rural Utilities Service (RUS), the West Virginia Infrastructure and Jobs Development Council (IJDC), the United States Department of Commerce, Economic Development Administration (EDA), the Appalachian Regional Commission (ARC), and the West Virginia Bureau of Public Health. All work shall be performed in accordance with the regulations issued by the federal funding source and the State of West Virginia pertaining thereto. The selected firm will be required, if applicable, to comply with Title VI of the Civil Rights Act of 1964, President's Executive Order 11246, Section 109 of the Housing and Urban Development Act of 1974, Section 3 of the Housing and Urban Development Act of 1968, Conflict of Interest Statement, Access to Records provisions, and the Uniform Relocation Assistance and Real Property Acquisitions Policies Act of 1970. The selected firm must also fully comply with the Minority/Women-Owned Business Enterprise and Affirmative Action requirements. If a portion of the project is subcontracted, then "positive efforts" must be made to subcontract to Minority/Women-Owned Businesses. This shall be done in compliance with the six (6) affirmative steps as outlined in 40 CFR 33.240.

The Town of New Haven will afford full opportunity to minority business enterprises to respond to this request and will not discriminate against any interested firm or individual on the grounds of race, creed, color, sex, age, handicap or national origin in the awarding of this contract.

Attention is directed to the fact that the Town of New Haven will not enter into an agreement for engineering services for this project unless, in the Town's opinion, funding is imminent or actually received which is sufficient to allow the Town to complete the project. Notice is hereby given that consulting engineering services will be utilized for the preparation of construction contract plans and related documents for a pedestrian footbridge on the Potts Valley Rail Trail, Monroe County, West Virginia. The work will require the creation of said architectural plans and build specs to enable the construction of a single-span footbridge of between 20 and 30 feet in length, and sufficient in width to accommodate foot travel including those transporting bicycles across the stream. The general appearance of the footbridge should resemble a small vintage wooden railway trestle, and the design may need to incorporate a slight curve. Ongoing dialog during the design process must be maintained with the U. S. Forest Service, Eastern Divide District Office, Blacksburg, VA, or their designee. Reasonably prompt completion of the work will be required.

The structure will be built at the Crosier Branch crossing of said trail; roughly 1.5 miles distant from the NE trailhead, at coordinates 37° 27.545' N, 80° 26.205' W. At least one site visit, and more as necessary, should be made by the selected firm prior to the completion of the construction plans. Design consideration should be given to the fact that the construction site is currently removed from any road access, and access created to allow for transport of building materials will likely be primitive in nature.

5. A cost accounting information statement indicating whether a cost accounting system has been maintained and is in effect such that is capable of segregating and identifying accumulating costs for each job that is performed under cost type projects.

A single letter of interest and unpriced prospectus is required. All evaluation factors will be given equal consideration in the selection process. Prior to entering into contract negotiations, the selected firm and any sub-consultants shall submit a company computed overhead rate for review.

There is also conveyed to the party of the second part the following appurtenant easements:

(B) Non-exclusive easements for encroachments upon the common area or upon other residence units, for the portions of the building constituting said condominium, resulting from settling or any cause, other than the willful act of the party of the second part, his heirs or assigns.

This conveyance is made subject, however, to the following reservations:

(1) Any portion of the common area lying within said residence unit.

(2) An easement appurtenant to the common area and all other residence units for encroachment resulting from settling or other causes except the willful acts of the residence unit owners.

(3) An easement on behalf of the Council of Hunters Ridge Owners Association, Inc., to enter said residence unit, to maintain, repair or replace common elements, as well as to make repairs to such residence unit if such repairs are reasonably necessary for public safety or to prevent damage to other residence units or to the common elements.

(4) The Declaration for Hunters Ridge, a Condominium dated June 29, 1977, and of record in the aforesaid Clerk`s Office in Deed Book 1837, at page 356, and as later amended.

(5) The Code of Regulations for Hunters Ridge Condominium dated June 29, 1977, of record in the aforesaid Clerk`s Office in Deed Book 1837, at page 403, (hereinafter called "Code").

The parties of the second part for themselves and on behalf of their heirs, devisees, personal representatives, successors and assigns, by the acceptance of this deed and by joining in the execution of same: (1) covenant and agree to be bound by and comply with all of the covenants, terms, provisions and conditions set forth in the Declaration; (2) covenant and agree to pay such charges for the maintenance of, repairs to, replacement of, and expenses in connection with the common elements as may be assessed from time to time by the Council of the said Hunters Ridge Owners Association, Inc., in accordance with the Code and Unit Property Act of West Virginia; and (3) further covenant and agree that the residence unit conveyed by this deed shall be subject to a charge for all amounts so assessed and that, except insofar as Section 5, Article VII of Chapter 36A of the Code of West Virginia of 1931, as amended (Unit Property Act) may relieve a subsequent residence unit owner of liability for prior unpaid assessments, this covenant shall run with and bind the land or the residence unit hereby and all subsequent owners thereof, and irrevocably appoint Hunters Ridge, Inc., a corporation, its successors and assigns, as their attorney-in-fact, coupled with an interest, to give, execute and record the consent of the said party of the second part to any and all amendments to the Declaration reserved to Hunters Ridge, Inc., in said Declaration.

This conveyance is made expressly subject further to all rights-of-way and easements and the reservations, covenants and provisions as contained or referred to in a deed from Estill & Greenlee, Inc., a corporation, to Hunters Ridge, Inc., a corporation, dated June 14, 1976, and recorded in the Clerk`s Office in Deed Book 1797, at page 261. It is understood and agreed that the aforesaid deed dated June 14, 1976, from Estill & Greenlee, Inc., a corporation, to Hunters Ridge, Inc., a corporation, conveyed only the surface of property therein described, the coal in and underlying said property having been severed by deed from D. G. Courtney to Fred B. Lott and W. F. Jacoby, dated October 11, 1901, and recorded in the Clerk`s Office in Deed Book 86, at page 444, to which deed reference is herewith made for a further and fuller description of the terms of said conveyance, including the rights, privileges and appurtenances thereunto set forth. By deed dated June 10, 1902, and recorded in the Clerk`s Office in Deed Book 86, at page 451, D. G. Courtney conveyed to A.W. Lively all of the same real estate that was conveyed to Hunters Ridge, Inc., a corporation, by the aforesaid deed dated June 14, 1976, which deed dated June 10, 1902, reserved from the operation thereof all minerals and mineral substances, together with certain rights, privileges and appurtenances, all as therein set forth, to which deed reference is hereby made for a further and fuller description thereof.

This conveyance is made subject to any and all covenants, restrictions, and easements as the same may appear in instruments of record in the aforesaid Clerk`s Office.
