The Monongahela National Forest is advertising the prospectus for a special use permit to operate and maintain a boat rental concession business at the Lake Sherwood Recreation Area on the Marlinton/White Sulphur Ranger District, Greenbrier County, WV. Bids will be received by the Marlinton/White Sulphur District Ranger, Monongahela National Forest, Marlinton, WV, from December 1, 2014 until 2:00p.m. Eastern Time January 30, 2015.
The permit issued to the successful bidder will authorize the use of an area adjacent to the public boat dock located at Lake Sherwood. The area is approximately 0.25 acres, and includes shoreline, boat rental building, boat storage racks, boat ramp and dock, parking area, shade structure, bench, vault toilet, flush toilet, and associated grounds.
A prospectus which includes bidding procedures, a description of the area and business opportunity, and the special use permit, is available upon request from:

REISSUANCE OF REVENUE BONDS
(CHARLESTON MEDICAL CENTER HOUSING
CORPORATION PROJECT)
SERIES 2007
AND RELATED MATTERS

Pursuant to Section 8-16-7 of the Code of West Virginia of 1931, as amended, a public hearing will be held by the Charleston Building Commission (the "Building Commission") on the following entitled Ordinance and on the question of whether there should be approved the reissuance for federal tax purposes of the Revenue Bonds (Charleston Medical Center Housing Project), Series 2007 (the "Bonds") issued by the Building Commission on June 29, 2007 in an aggregate principal amount of $2,500,000 for the purpose of reducing the interest rate on the Bonds for federal income tax purposes (the "Reissuance") to effect a savings. The Facilities (as hereinafter defined) will continue to be owned and used by the Charleston Medical Center Housing Corporation, a West Virginia nonprofit corporation, described in Section 501(c)(3) of the Code and exempt from federal income taxation under Section 501(a) of the Code. At the special meeting of the Board of the Building Commission to be held at 10:00 a.m., on November 19, 2014, in its office at 815 Quarrier Street, Suite 244, Charleston, West Virginia and at such hearing all parties and interested persons may attend and appear before the Building Commission and be heard as to whether the Ordinance shall be put into effect and to express their views on the proposed reissuance of the Bond and at such hearing all objections and suggestions shall also be heard and the Building Commission shall then take such action as it shall deem proper in the premises upon an Ordinance entitled:

ORDINANCE AUTHORIZING THE REISSUANCE FOR FEDERAL TAX PURPOSES OF CHARLESTON BUILDING COMMISSION REVENUE BONDS (CHARLESTON MEDICAL CENTER HOUSING CORPORATION PROJECT) SERIES 2007 ISSUED IN THE ORIGINAL AGGREGATE PRINCIPAL AMOUNT OF $2,500,000 TO REDUCE THE INTEREST RATE TO EFFECT A SAVINGS, WHICH BONDS PROVIDED FUNDS TO FINANCE THE COSTS OF DESIGNING, ACQUIRING, CONSTRUCTING AND ERECTING HOUSING FACILITIES FOR STUDENTS OF INSTITUTIONS OF HIGHER EDUCATION; AUTHORIZING THE EXECUTION AND DELIVERY OF THE BONDS; DESIGNATING THE BONDS AS "BANK-QUALIFIED;" AUTHORIZING THE EXECUTION AND DELIVERY OF OTHER DOCUMENTS, INSTRUMENTS, AGREEMENTS AND CERTIFICATES AND AUTHORIZING THE TAKING OF ALL OTHER ACTIONS RELATING TO SUCH REISSUANCE.

The above-entitled Ordinance was read on first reading by the Building Commission on November 5, 2014. The above-quoted title of the Ordinance generally describes the contents thereof and the purposes of the bond issue contemplated thereby. The proceeds of the Bonds were used to finance the costs of (i) designing, acquiring, constructing and equipping a 24-unit housing facility located at the corner of Morris and Quarrier Streets, Charleston, West Virginia, for occupancy by students and medical residents receiving clinical training and education through their respective institutions of higher education at Charleston Area Medical Center, Inc. (the "Facilities"); and (ii) paying the costs of issuance of the Bonds and related costs (collectively, the "Project"). The site is leased by the Charleston Medical Center Housing Corporation (the "Corporation") to the Building Commission, which then leased the property back to the Corporation. The rentals paid by the Corporation under the lease are sufficient to pay the principal of and interest due on the Bonds. The Bonds are payable solely from revenues and rentals to be derived from the leasing of the property by the Corporation. No taxes may be levied at any time for the payment of the Bond or the interest thereon.

A certified copy of the above-entitled Ordinance is on file at the office of the Building Commission, 815 Quarrier Street, Suite 244, Charleston, West Virginia, for review by interested persons during regular office hours.

Following the public hearing, the Ordinance will become effective.

Billing information must line up with preprinted areas of the invoice to be determined by the City of Nitro consistent with the billing data provided by the city. The city will also provide information each month for the center section of the invoice, that will require the printer to set up and layout with clip art providing citizens with information on upcoming events.
Delivery must be made to the Nitro Post Office within three (3) days after receipt of data files.
Vendor will accept WV Purchasing Card.

Sealed bids for the construction of Contract No. 1: Midelburg Sewer Separation Project will be received by the City of Logan until 11:00 A.M., L.P.T., December 16, 2014 at the City of Logan's Office located at 219 Dingess Street, Logan County, WV 25601 for furnishing labor,
materials, and performing all the work set forth in the Form of Proposal. Bids will be opened at 11:00 A.M., L.P.T., December 16, 2014. Immediately following the scheduled closing time for the reception of bids, all proposals which have been submitted in accordance with the conditions of this project will be publicly opened and read aloud.

The work to be bid upon is described as follows: CONTRACT NO. 1 - consists of approximately 240 LF of 10" PVC gravity sewers replacement of 2 existing manholes, modifications to manhole N-5, connection to existing sewer lines, cut and cap existing overflow, installation of storm manhole
and reconnection of service lines, concrete trench repair, restoration, asphalt trench repair and asphalt overlay, site sign and all other appurtenances shown on the plans or detailed in the specifications that
are necessary for a complete and operable sanitary and storm sewerage system.

The total construction time for Contract No. 1 is limited to sixty (60) consecutive calendar days from the date specified in the contract documents.

Plans and Specifications are available at the offices of Dunn Engineers, Inc., 400 South Ruffner Rd, Charleston, WV, upon payment of one hundred fifty dollars ($150.00) for the purchase of Contract No. 1, which is not subject to refund. All Bidders must purchase plans and bid forms from the Engineer in order to bid. Bidders wishing to have plans mailed shall provide their own Federal Express or UPS account number for shipping charges to be billed directly to their own account.

Envelope No. 1 will be opened first and the Bid Bond, Power of Attorney, Certificate of Compliance and other pertinent/required documents as listed on the "Bidder's Opening Requirements Checklist" will be checked to make sure that all documents are in order. If the documents are found to be in order, Envelope No. 2 containing the Form of Proposal will then be opened and publicly read aloud.

If the documents from Envelope No. 1 are not in order, Envelope No. 2 containing the Form of Proposal will not be opened and the bid will be considered non-responsive and will be returned to the Bidder.

The successful low bidder will be provided four (4) complete set of plans and specifications at no additional cost after the contract has been signed. Any additional sets of plans and detail specifications that are requested will be provided at a cost of: one hundred fifty dollars ($150) per set. Payment by either cash or check must be made upon delivery.

A Bidder may not withdraw his bid for a period of ninety (90) days after the date set for the opening of bids.

All Bidders are required to have a West Virginia Contractor's License as per Section 21-11-11 of the WV Code. The prospective bidder's license number must be included in the Envelope No. 1 as per the Bid Opening Checklist. All subcontractors shall furnish their WV Contractor's License number to the prime contractor prior to award of the contract.

Bids shall be accompanied by a certified check or bid bond payable to the City of Logan, Logan County, WV, in an amount not less than five percent (5%) of the base bid.

Bidders must hold a current West Virginia Contractor's License on the date of Bid Opening.

The City of Logan reserves the right to reject any and all bids

DRUG FREE WORKPLACE

West Virginia Code 21-1D-5 provides that any solicitation for a public improvement construction contract requires each vendor that submits a bid for the work to submit at the same time an affidavit of compliance with the bid. The enclosed Drug-Free Workplace Affidavit must be signed and
submitted in the first envelope as evidence of the vendor's compliance with the provisions of Article 1D, Chapter 21 of the West Virginia Code. Failure to submit the signed Drug-Free Workplace Affidavit in the First Envelope shall result in disqualification of such bid.

LIQUIDATED DAMAGES

Bidder maybe assessed liquidated damages if construction exceeds the allotted contract time. The amount of the damages shall be in accordance with the Contract Documents.

NONDISCRIMINATION IN EMPLOYMENT

Bidders on this work will be required to comply with the President's Executive Order 11246. The requirements for Bidders and Contractors under this order are explained in the Specifications. The low responsive Bidder is required to submit to EPA a statement in accordance with Executive Order 11246.

SUSPENSION AND DEBARMENT

Recipient shall not entertain the use of businesses that are listed on the "Excluded Parties List System at www.epls.gov in accordance with 2 CFR Part 1532 and Subpart B and C of 2 CFR Part 180.

NOTICE OF FEDERAL AND STATE WAGE REQUIREMENTS

Contractors are required to comply with all laws pertaining to prevailing wage rates for the State of West Virginia as issued by the Division of Labor and the federal wage rates issued in accordance with the Davis Bacon Act. Bids shall be submitted in accordance with instructions in Specifications, and each bid shall be accompanied by a bid bond, made payable to Raleigh County Board of Education, in the amount of five percent (5%) of the base bid, with the understanding that it shall guarantee that the bidder will not withdraw his bid for a period of sixty (60) days and that if his bid is accepted, bidder will enter into formal contract with Owner, that the required bond will be given, and that in the event of the withdrawal of said bid within the time specified, the bidder shall be liable to the Owner on account of default of the bidder in any particular thereof. The bid bonds of all unsuccessful bidders will be returned when contract and bond have been executed.
All Prime Contractors and Subcontractors shall be licensed in compliance with West Virginia Code Chapter 21, Article 11 (Contractor Licensing Act). All Prime contractors shall include their West Virginia Contractor License Number in their Bid Proposal. The successful Bidder will be required to execute a contract, comply with all rules and regulations of the West Virginia Workers' Compensation Laws, and provide evidence of insurance coverage to the minimum limits required by the contract documents.
The bidder to whom the contract is awarded shall execute and deliver to the Owner within ten (10) days after the award, and before signing the contract, a corporate payment and performance bond in a penal sum equal to at least the contract sum and the cost of such a bond shall be paid by the bidder. No bid may be withdrawn after the scheduled closing time for receipt of bids for at least ninety days. Other requirements are detailed in the bid package.
The Owner reserves the right to reject any or all bids, and the right to withhold final award of contract for sixty (60) days after opening bids.

Notice is hereby given that consulting services will be utilized by the West Virginia Department of Transportation, Division of Highways, for Quality Assurance Management (QAM) services and related duties as required for the upgrade of US 35 from WV 869 to Mason CR 40 in Putnam and Mason Counties, West Virginia. This project is being developed as a Public-Private-Partnership utilizing Design-Build procurement.

The QAM will be responsible for all quality assurance duties normally performed by the WVDOH in regards to materials certification, construction inspection, and design reviews. The QAM will certify all materials and inspection on the project and provide record keeping to document certifications. QAM shall provide an independent certified lab for testing of materials. QAM will act on DOH's behalf in the review of all plan submissions. All work will be performed in accordance with WVDOH Standard Specifications (2010), as amended by the Supplemental Specifications (2014), WVDOH Construction Manual (2002), and current Materials Procedures. Anticipated duration of the agreement is approximately 48 months. It is anticipated that appropriately accredited and qualified technicians, Levels I-V, for the area of assignment may be required. Plan reviews and general oversight of these services will be performed by a Professional Engineer(s) licensed in the state of West Virginia.

QAM shall also be responsible for reviews of schedules and progress payments; field survey verification; assurance of environmental compliance; project record keeping including oversight of as-built plans preparation; and related duties as required. QAM shall provide all tools and equipment needed to perform these tasks.

The general guidelines for performance of the project inspection are as follows:

1) The consulting engineering services required may consist of all quality assurance tasks, documentation services, and related duties as required.

2) The consultant will supply experienced construction and materials inspection personnel. All inspectors must be certified through BridgeValley (formerly Bridgemont) Community and Technical College.

3) The personnel shall have a working knowledge of WVDOT-DOH standards, specifications and materials testing procedures as well as being certified by the DOH for sampling and testing aggregate, portland cement concrete, compaction testing and possess nuclear certifications in the State of West Virginia. The knowledge and certifications required of a given individual will depend on the specific item(s) being inspected and /or sampled/tested.

4) It is the intent that the majority of the required materials testing be conducted by personnel capable of performing the complete field inspection of the affected items.

5) In addition the inspectors shall be knowledgeable of good construction practices and surveying practices and procedures; able to interpret contracts, special provisions, plans and specifications; able to exercise good judgment; able to use engineering, test and office equipment; be accurate and neat in recording data and preparing records and able to maintain a harmonious relationship with the Department, other governmental agencies, contractors and the public.

6) The consultant will supply all necessary transportation for its personnel.

7) The Department will supply field offices, storage buildings and office equipment per normal construction contract specification items or reasonable access to same as it deems necessary.

8) The Department will familiarize appropriate Consultant personnel with the use of its Site Manager and ProjectWise software for preparation and maintenance of project records and documentation and supply Department required forms to the Consultant. However, the QAM is expected to provide and maintain the appropriate version of Primavera P6 Scheduling software and also the experience level to afford proper review of the Contract Schedule.

9) All testing equipment and incidental tools and supplies are to be furnished by the Consultant unless the Department elects to supply same on a given project.

10) The method of payment for the consultant services provided will be specific rate of pay.

Firms interested in being considered for this work must submit a letter of interest and unpriced prospectus to Mr. Jason M. Boyd, Director, Contract Administration Division, West Virginia Department of Transportation, Division of Highways, Building Five, Room A-722, 1900 Kanawha Boulevard, East, Charleston, West Virginia 25305 prior to 4:00 P.M. on December 5, 2014. Responses received after this time will not be considered.

As a minimum, the letter of interest will include the following (submitted in duplicate):

1) A completed copy of the Division's Consultant Engineer Confidential Qualification Questionnaire (unless already on file with the Division). The questionnaire should be current as of January 1, 2014 or later.

6) Indication of whether a cost accounting system has been maintained and is in effect such that it is capable of segregating and identifying accumulating costs for each job that is performed under cost-type contracts.

Submittals shall be prepared simply and economically, providing a straight forward, concise description of the firm's capabilities to satisfy the requirements of the RFP. Emphasis should be on completeness and clarity of content. Elaborate brochures and other representations beyond that sufficient to present a complete and effective proposal are neither required nor desired
Prior to entering into contract negotiations, the selected firm and any subconsultants shall submit a current audited overhead rate computation performed by a State or Federal government agency or independent CPA firm. The submitted overhead audit shall contain a detailed exhibit of the computations with all applicable eliminations and references. The audit shall be done in accordance with Government Auditing Standards issued by the Comptroller General of the United States and all eliminations required by the Federal Acquisition Regulations will be made.

3) The personnel shall have a working knowledge of WVDOT-DOH standards, specifications and materials testing procedures as well as being certified by the DOH for sampling and testing aggregate, portland cement concrete, bituminous concrete, compaction testing and possess nuclear certifications in the State of West Virginia. The knowledge and certifications required of a given individual will depend on the specific item(s) being inspected and /or sampled/tested.

4) It is the intent that the majority of the required materials testing be conducted by personnel capable of performing the complete field inspection of the affected items.

5) In addition the inspectors shall be knowledgeable of good construction practices and surveying practices and procedures; able to interpret contracts, special provisions, plans and specifications; able to exercise good judgment; able to use engineering, test and office equipment; be accurate and neat in recording data and preparing records and able to maintain a harmonious relationship with the Department, other governmental agencies, contractors and the public.

6) The consultant will supply all necessary transportation for its personnel.

7) The Department will supply field offices, storage buildings and office equipment per normal construction contract specification items or reasonable access to same as it deems necessary.

8) The Department will familiarize appropriate Consultant personnel with the use of its Site Manager and ProjectWise software for preparation and maintenance of project records and documentation and supply Department required forms to the Consultant. However, the QAM is expected to provide and maintain the appropriate version of Primavera P6 Scheduling software and also the experience level to afford proper review of the Contract Schedule.

9) All testing equipment and incidental tools and supplies are to be furnished by the Consultant unless the Department elects to supply same on a given project.

10) The method of payment for the consultant services provided will be specific rate of pay.

Firms interested in being considered for this work must submit a letter of interest and unpriced prospectus to Mr. Jason M. Boyd, Director, Contract Administration Division, West Virginia Department of Transportation, Division of Highways, Building Five, Room A-722, 1900 Kanawha Boulevard, East, Charleston, West Virginia 25305 prior to 4:00 P.M. on December 5, 2014. Responses received after this time will not be considered.

As a minimum, the letter of interest will include the following (submitted in duplicate):

1) A completed copy of the Division's Consultant Engineer Confidential Qualification Questionnaire (unless already on file with the Division). The questionnaire should be current as of January 1, 2014 or later.
2) A completed copy of the "Consultant Short List Selection Criteria Technical Evaluation" form found in Appendix E of the WVDOT-DOH Manual for Consultant Services or from the WVDOT web homepage by selecting the following tabs: Business/Upcoming Consultant Contracts/Engineering/Forms and Documents http://www.transportation.wv.gov/highways/engineering/Pages/FormsandDocuments.aspx

6) Indication of whether a cost accounting system has been maintained and is in effect such that it is capable of segregating and identifying accumulating costs for each job that is performed under cost-type contracts.

Submittals shall be prepared simply and economically, providing a straight forward, concise description of the firm's capabilities to satisfy the requirements of the RFP. Emphasis should be on completeness and clarity of content. Elaborate brochures and other representations beyond that sufficient to present a complete and effective proposal are neither required nor desired
Prior to entering into contract negotiations, the selected firm and any subconsultants shall submit a current audited overhead rate computation performed by a State or Federal government agency or independent CPA firm. The submitted overhead audit shall contain a detailed exhibit of the computations with all applicable eliminations and references. The audit shall be done in accordance with Government Auditing Standards issued by the Comptroller General of the United States and all eliminations required by the Federal Acquisition Regulations will be made.

ROUTE 33 SEWER PROJECT
PUTNAM COUNTY

Putnam Public Service District has completed the installation of sanitary sewer lines in the Route 33 areas of Putnam County.

1. Service is available as of November 3, 2014

2. The District will begin mandatory billing for sewer service in December, 2014 as authorized by West Virginia Code §16-13A9 and Rule 4.3 of the Public Service Commission of West Virginia Rules for the Government of Sewer Utilities. Billings shall be based on actual water consumption.

3. The District's tariff upon completion of all construction from the
Sewer Project is as follows:

$ 11.20 per 1,000 gallons
of water used
$ 22.40 minimum bill
$ 33.59 flat rate for non-metered and users who obtain water from wells

This rate has been approved by the Public Service Commission of West Virginia, Case No. 12-1209-PSD-CN.

4. West Virginia Code §16-13A9 authorizes the District to petition the
Circuit court of Putnam County to compel connection to the sewer system.

5. Residential customers that do not connect to the sewer system will be
billed on actual water consumption, up to 4,500 gallons, starting with the service dates of December, 2014. These charges will reflect on your February billing.

6. All persons in the project area who have not made a request for service with the Putnam Public Service District's must do so at this time. The main office is located at 107 South Poplar Fork Road, Scott Depot, next to the Teays Valley Volunteer Fire Department. The District's telephone number is 757-6551.

The Pocahontas County Commission is requesting bids for a trail maintenance project within the Tea Creek Trail System located adjacent to the Highland Scenic Highway within the Monongahela National Forest in Pocahontas County. The project will include building approximately one (1) mile of new trail, improving drainage by installing such features as drainage dips, geosynthetics and hardening of trail tread utilizing local materials on four (4) miles of existing trails. The use of both mechanical equipment and hand tools will be required to complete this project. The bid should include an estimate package for the project including materials and labor. All trail infrastructure, newly constructed or upgraded by maintenance activities, must be compliant with the "US Forest Service Trail Accessibility Guidelines".

Wages paid for all work must be in accordance with West Virginia prevailing wage rates. Successful bidder must also possess a valid West Virginia contractor's license and liability insurance, and will be required to post a performance bond.

All bids should be submitted in writing and placed in a sealed envelope marked "Tea Creek Area Trails Project" and delivered by mail or in person to the Pocahontas County Commission, at 900 Tenth Avenue, Marlinton, West Virginia, on later than 4:30 pm. on Friday, October 31, 2014. ADVERTISEMENT FOR BIDS

Sealed Bids for the construction of the Contract #1 - Wastewater Treatment Plant Upgrade will be received, Town of Masontown, at the office of the City Hall, 27 West Midway Avenue, Masontown, West Virginia 26542 until 2:00 pm local time on December 16, 2014, at which time the Bids received will be publicly opened and read.
The approximate quantities of work to be bid upon are described as follows:

The wastewater treatment plant upgrade includes construction of a new operations building that consists of a mechanical bar screen with screenings processing, vortex grit removal system with classifier, two (2) sequencing batch reactors (SBR's), cloth media filters, chemical feed equipment and holding tanks, ultraviolet disinfection equipment, polymer feed equipment, sludge holding tanks, belt filter press, lime feed and mixing equipment, solids conveying system and emergency standby generator with automatic transfer switch. Site work shall include paving, grading, piping, sidewalks, fencing, bollards, and all other work related items. The work shall include all ancillary mechanical, electrical, instrumentation and controls, SCADA, spare parts and accessories, civil, structural, architectural, and other items needed to provide a complete fully tested and operable system.

The Work will be substantially completed within 420 calendar days after the date when the Contract Times commence to run, and completed and ready for final payment within 480 days after the date when the Contract Times commence to run. Liquidated damages shall be $1,000.00 per day.

The West Virginia Department of Environmental Protection has approved a cleanup method to address environmental conditions at the former Kelly Field Site under the Voluntary Remediation and Redevelopment Act. The applicant, Union Carbide Corporation, has been working with the WVDEP since negotiating a Voluntary Remediation Agreement (VRA) on February 28, 2005.
The site is located at 21st Street, North Charleston, Kanawha County, West Virginia, and is comprised of approximately 7.25 acres of property. Union Carbide Corporation operated a warehouse and drum storage facility at the site from the late 1940s to the early 1990s.
The cleanup method includes the installation of a soil cover and institutional controls. Upon completion of remediation, the applicant will submit a final report to OER to confirm that the work meets all applicable remediation standards.

The institutional controls will include restrictions to prohibit extraction of groundwater at the site for any use except groundwater monitoring and/or remediation, limit use to non-residential, require the soil cover to be maintained, prohibit subsurface and/or earthmoving activities unless it is completed by persons qualified and knowledgeable about the contaminants at the site and the work is performed in accordance with a Health and Safety Plan and a soil management plan, require vapor mitigation technologies or further evaluation of the vapor intrusion pathy to establish whether mitigation is needed for all future enclosed buildings intended for human occupancy.

Notice is hereby given that consulting services will be utilized by the West Virginia Department of Transportation, Division of Highways, for Quality Assurance Management (QAM) services and related duties as required for the upgrade of US 35 from WV 869 to Mason CR 40 in Putnam and Mason Counties, West Virginia. This project is being developed as a Public-Private-Partnership utilizing Design-Build procurement.

The QAM will be responsible for all quality assurance duties normally performed by the WVDOH in regards to materials certification, construction inspection, and design reviews. The QAM will certify all materials and inspection on the project and provide record keeping to document certifications. QAM shall provide an independent certified lab for testing of materials. QAM will act on DOH's behalf in the review of all plan submissions. All work will be performed in accordance with WVDOH Standard Specifications (2010), as amended by the Supplemental Specifications (2014), WVDOH Construction Manual (2002), and current Materials Procedures. Anticipated duration of the agreement is approximately 48 months. It is anticipated that appropriately accredited and qualified technicians, Levels I-V, for the area of assignment may be required. Plan reviews and general oversight of these services will be performed by a Professional Engineer(s) licensed in the state of West Virginia.

QAM shall also be responsible for reviews of schedules and progress payments; field survey verification; assurance of environmental compliance; project record keeping including oversight of as-built plans preparation; and related duties as required. QAM shall provide all tools and equipment needed to perform these tasks.

The general guidelines for performance of the project inspection are as follows:

1) The consulting engineering services required may consist of all quality assurance tasks, documentation services, and related duties as required.

2) The consultant will supply experienced construction and materials inspection personnel. All inspectors must be certified through BridgeValley (formerly Bridgemont) Community and Technical College.

3) The personnel shall have a working knowledge of WVDOT-DOH standards, specifications and materials testing procedures as well as being certified by the DOH for sampling and testing aggregate, portland cement concrete, compaction testing and possess nuclear certifications in the State of West Virginia. The knowledge and certifications required of a given individual will depend on the specific item(s) being inspected and /or sampled/tested.

4) It is the intent that the majority of the required materials testing be conducted by personnel capable of performing the complete field inspection of the affected items.

5) In addition the inspectors shall be knowledgeable of good construction practices and surveying practices and procedures; able to interpret contracts, special provisions, plans and specifications; able to exercise good judgment; able to use engineering, test and office equipment; be accurate and neat in recording data and preparing records and able to maintain a harmonious relationship with the Department, other governmental agencies, contractors and the public.

6) The consultant will supply all necessary transportation for its personnel.

7) The Department will supply field offices, storage buildings and office equipment per normal construction contract specification items or reasonable access to same as it deems necessary.

8) The Department will familiarize appropriate Consultant personnel with the use of its Site Manager and ProjectWise software for preparation and maintenance of project records and documentation and supply Department required forms to the Consultant. However, the QAM is expected to provide and maintain the appropriate version of Primavera P6 Scheduling software and also the experience level to afford proper review of the Contract Schedule.

9) All testing equipment and incidental tools and supplies are to be furnished by the Consultant unless the Department elects to supply same on a given project.

10) The method of payment for the consultant services provided will be specific rate of pay.

Firms interested in being considered for this work must submit a letter of interest and unpriced prospectus to Mr. Jason M. Boyd, Director, Contract Administration Division, West Virginia Department of Transportation, Division of Highways, Building Five, Room A-722, 1900 Kanawha Boulevard, East, Charleston, West Virginia 25305 prior to 4:00 P.M. on December 5, 2014. Responses received after this time will not be considered.

As a minimum, the letter of interest will include the following (submitted in duplicate):

1) A completed copy of the Division's Consultant Engineer Confidential Qualification Questionnaire (unless already on file with the Division). The questionnaire should be current as of January 1, 2014 or later.

STATEWIDE QUALITY ASSURANCE
MANAGEMENT

Notice is hereby given that consulting services will be utilized by the West Virginia Department of Transportation, Division of Highways, for Quality Assurance Management (QAM) services and related duties as required on various Design-Build projects throughout the State. The anticipated duration of the agreement is twelve months coincident with the calendar year 2015 with the potential for a (mutually agreeable) twelve month extension. Multiple consultants will be selected.

The QAM will be responsible for all quality assurance duties normally performed by the WVDOH in regards to materials certification, construction inspection, and design reviews. The QAM will certify all materials and inspection on the project and provide record keeping to document certifications. QAM shall provide an independent certified lab for testing of materials. QAM will act on DOH's behalf in the review of all plan submissions. All work will be performed in accordance with WVDOH Standard Specifications (2010), as amended by the Supplemental Specifications (2014), WVDOH Construction Manual (2002), and current Materials Procedures. It is anticipated that appropriately accredited and qualified technicians, Levels I-V, for the area of assignment may be required. Plan reviews and general oversight of these services will be performed by a Professional Engineer(s) licensed in the state of West Virginia.

QAM shall also be responsible for reviews of schedules and progress payments; field survey verification; assurance of environmental compliance; project record keeping including oversight of as-built plans preparation; and related duties as required. QAM shall provide all tools and equipment needed to perform these tasks.

The general guidelines for performance of the project inspection are as follows:

1) The consulting engineering services required may consist of all quality assurance tasks, documentation services, and related duties as required.

2) The consultant will supply experienced construction and materials inspection personnel. All inspectors must be certified through BridgeValley (formerly Bridgemont) Community and Technical College.

3) The personnel shall have a working knowledge of WVDOT-DOH standards, specifications and materials testing procedures as well as being certified by the DOH for sampling and testing aggregate, portland cement concrete, bituminous concrete, compaction testing and possess nuclear certifications in the State of West Virginia. The knowledge and certifications required of a given individual will depend on the specific item(s) being inspected and /or sampled/tested.

4) It is the intent that the majority of the required materials testing be conducted by personnel capable of performing the complete field inspection of the affected items.

5) In addition the inspectors shall be knowledgeable of good construction practices and surveying practices and procedures; able to interpret contracts, special provisions, plans and specifications; able to exercise good judgment; able to use engineering, test and office equipment; be accurate and neat in recording data and preparing records and able to maintain a harmonious relationship with the Department, other governmental agencies, contractors and the public.

6) The consultant will supply all necessary transportation for its personnel.

7) The Department will supply field offices, storage buildings and office equipment per normal construction contract specification items or reasonable access to same as it deems necessary.

8) The Department will familiarize appropriate Consultant personnel with the use of its Site Manager and ProjectWise software for preparation and maintenance of project records and documentation and supply Department required forms to the Consultant. However, the QAM is expected to provide and maintain the appropriate version of Primavera P6 Scheduling software and also the experience level to afford proper review of the Contract Schedule.

9) All testing equipment and incidental tools and supplies are to be furnished by the Consultant unless the Department elects to supply same on a given project.

10) The method of payment for the consultant services provided will be specific rate of pay.

Firms interested in being considered for this work must submit a letter of interest and unpriced prospectus to Mr. Jason M. Boyd, Director, Contract Administration Division, West Virginia Department of Transportation, Division of Highways, Building Five, Room A-722, 1900 Kanawha Boulevard, East, Charleston, West Virginia 25305 prior to 4:00 P.M. on December 5, 2014. Responses received after this time will not be considered.

As a minimum, the letter of interest will include the following (submitted in duplicate):
